Enhanced Focus on Traumatic Bleeding in UK First Aid

Recent updates to first aid training standards in the UK, effective October 2025, are placing a significant new emphasis on the management of traumatic (catastrophic) bleeding. These changes, falling under the Health and Safety (First-Aid) Regulations 1981, mandate that all first aid training aligns with current best practices from recognised bodies such as the Resuscitation Council UK (RCUK) and Voluntary Aid Societies including St John Ambulance and the British Red Cross.

Previously, advanced bleeding control techniques were often reserved for higher-level emergency training. However, the updated guidelines acknowledge that life-threatening bleeding can occur in any setting, from care homes to typical workplaces. This means that first aid training will now extend beyond basic wound management to include skills such as identifying and responding to severe bleeds, applying firm direct pressure, and the appropriate use of tourniquets or haemostatic dressings. The Resuscitation Council UK (RCUK) advises that for life-threatening extremity bleeding not controlled by direct manual pressure, a tourniquet should be applied 5-7cm above the injury, but not over a joint, and tightened until bleeding stops. The time of application should be noted, and the tourniquet should only be released by a healthcare professional.

Irish Workplace First Aid Regulations and Anaphylaxis Management

In Ireland, employers are legally required to provide adequate first aid equipment, facilities, and personnel to ensure employees receive immediate attention if injured or taken ill at work. The Safety, Health and Welfare at Work (General Application) Regulations 2007 outline these requirements, stressing the need for a thorough risk assessment to determine appropriate first aid provisions. This assessment should consider factors like the number of employees, the nature of work activities, and the proximity to emergency medical services.

The Health and Safety Authority (HSA) published updated Guidelines on First-Aid at Places of Work in late 2024, which align with the Pre-Hospital Emergency Care Council (PHECC) First Aid Response (FAR) training standard. The FAR course is the recognised standard for occupational first aiders in Ireland, covering advanced skills such as CPR, AED use, and managing serious injuries. Initial FAR training is an 18-hour classroom course, valid for two years, with a 12-hour recertification course required to maintain certification.

Adrenaline auto-injectors are recommended primarily for use by the general public for self-administration. While the recommended dose of adrenaline for teenagers and adults is 500 micrograms, most auto-injectors for adults deliver a 300 microgram dose. If symptoms do not resolve, IM adrenaline can be repeated after 5 minutes. Corticosteroids are no longer advised for the routine emergency treatment of anaphylaxis. Anaphylaxis is a serious, rapidly onset systemic hypersensitivity reaction that can be fatal, characterised by airway, breathing, or circulation problems. Deaths due to anaphylaxis are rare, estimated to occur in about one in two million people per year in Ireland, and are more commonly triggered by medications, blood transfusions, or insect stings rather than food allergy. However, a study found that in the UK, the case fatality rate for food-induced anaphylaxis more than halved from 0.7% in 1998 to 0.3% in 2018. Cow's milk was responsible for 26% of deaths in school-aged children in the UK between 1998 and 2018.

Choking Incidents and Prevention Strategies

Choking remains a significant concern across both the UK and Ireland. In the UK, between 2018 and 2022, 1399 deaths were caused by choking, with 1097 attributed to food substances and 302 to non-food objects. The elderly are identified as the most at-risk demographic due to factors such as eating too fast, difficulty chewing, and reduced saliva production. Young children are also particularly vulnerable, as they do not fully master chewing and swallowing until 4-6 years of age. A St John Ambulance study revealed that 34 children in the UK receive treatment for food choking every day.

In Ireland, HSE data from 2020-2024 reported over 3,000 choking incidents/near misses in HSE and HSE-funded services, with over 70% occurring in non-acute services such as care of the older person, mental health services, and disability services. More than 80% of these incidents were related to food and drink, and approximately 3% were medication-related. Twenty incidents resulted in death. The HSE highlights that difficulties with swallowing (dysphagia), often linked to conditions like dementia, stroke, or intellectual disability, significantly increase the risk of choking. In the European Union, an estimated 400 children aged 14 or younger die from choking each year.

Prevention strategies are crucial, especially for children. The HSE advises supervising children, keeping choking hazards out of reach, and ensuring toys are age-appropriate. Common non-food choking risks for children include small batteries, magnets, jewellery, coins, balloons, and small toy parts. For adults, particularly those with dysphagia, minimising risk involves providing appropriate diet consistencies, ensuring staff are trained in the International Dysphagia Diet Standardisation Initiative (IDDSI), and maintaining clear communication of dietary care plans.
